Your My AJH Record patient portal account includes information from care and services you receive at Anna Jaques Hospital.

Manage Your Health Information

You can use My AJH Record to manage your health care needs and those of your family. It is confidential, convenient and secure. Only you and your authorized proxies can view your information.

Use your My AJH Record patient portal 24/7 to:

• Download and share your information
• Give access to your records to a loved one
• Read educational materials tailored to your health concerns
• Review prescription(s)
• View hospital medical record(s)
• View imaging test results
• View laboratory test results

How to Begin Using My AJH Record

If you already have a My AJH Record account, visit the log in page and enter your username and password.

If you need to sign up for an account, the process is easy. Go to the My AJH Record home page and click the Sign up button.

You will need the following information to register for your patient portal account:

• Date of birth
• E-mail address
• Name
• Password
• Patient code (provided by your doctor)
• Two security questions
• Username

If you don’t have a patient code, call the Health Information Management Department at 978-463-1155 or visit our office at 25 Highland Ave. We are open Monday through Friday from 8:30 am to 4:00 pm (except holidays).

Once you confirm your identity, we can give you a patient code.
