Patient and Family Advisory Council
Our Patient Family Advisory Council (PFAC) provides a link between the hospital and the community, and to help identify the needs and priorities of patients and families from the communities we serve; Merrimack Valley Region and Southern New Hampshire. The Council assists AJH in creating an environment of patient and family centered care and improve quality and patient safety. It also provides the hospital feedback on potential solutions the hospital is considering implementing to solve a problem or improve care.
The Patient Family Advisory Council is designed to serve as a voice of the patient and family members and to encourage the partnership between patients, families, health care providers and the hospital.
Interested in Joining the PFAC?
Qualifications to become a member of the PFAC include being a current or recent AJH patient or family member and being interested in building a community partnership with the hospital. If you have an interest in helping to create an even better environment of patient and family centered care at Anna Jaques Hospital, please complete the PFAC Application.
Ways the PFAC helps:
- Identifying opportunities to improve the patient and family experience
- Providing input on hospital policies, practices and programs
- Sharing best practices across the hospital
