Persist Program for Pregnancy & Postpartum Substance Use Disorder

Supporting you and your baby

Supportive Care for Women in Recovery

If you are pregnant or a new mother in recovery, the Persist program at Anna Jaques Hospital can help you take control of your health.

Our team gives you the support you need during pregnancy and into your first year of motherhood.

Who We Serve

Our Persist program helps pregnant women who are:

  • Dealing with abuse or trauma.
  • In recovery and seeking additional support during pregnancy.
  • Living with a mental health disorder.
  • Managing a substance use disorder.

Your Patient Care Navigator

When you join the Persist program, you are paired with a patient care navigator. Your navigator helps manage your care. They are your advocate during your pregnancy and throughout your first year of parenthood.

Your navigator collaborates and meets with your care team during your pregnancy, labor and delivery, as well as after birth. It’s our goal to promote a healthy pregnancy and future for you and your baby. We do this by making these resources available to you:

  • Assessments and referrals to behavioral health providers
  • Connections to recovery support services
  • Referrals to helpful resources for you and your baby, such as free prenatal education
  • Referrals to local resources such as the Special Supplemental Nutrition Program for Women, Infants and Children (WIC)
  • Treatment for substance use in pregnancy

Meet Your Care Team

We know that you might be anxious about childbirth. That’s why your patient care navigator schedules time for you to visit the Birth Center and Neonatal Care Center at Anna Jaques. Together with your navigator, you take a tour and meet our team of doctors and nurses before your baby’s delivery. That way, you’ll feel more comfortable and prepared to welcome your baby.

Getting to Know Your Baby

After your baby is born, we follow the eat/sleep/console model for infant care, which allows you to remain with your baby. Our priority is to teach you how to comfort and care for your precious newborn, while we also care for your needs as a parent.

Your birth center care team doesn’t just show you how to care for your baby — we educate you and your partner on how to be a family. Your patient care navigator completes any referral paperwork you need and helps address any of your concerns.

If you or your baby needs other specialty services related to maternal substance use or any other condition, our team is happy to provide those services or make referrals as needed.
