Community Health Grants
Open Funding Opportunities
Each year, Anna Jaques Hospital offers several grants to groups and organizations working on issues identified in our needs assessments.
Those receiving our grants report on the outcomes and impacts of their projects at the conclusion of their grant period.
Our strategy is to support evidence-informed programs and services that reduce or address the community needs identified in our implementation strategy.

Anna Jaques Hospital Community Grants
Between 2024 and 2025, AJH is investing approximately $100,000 for its Community Grants Request for Proposals (RFP) to implement evidence based and/or evidence-informed strategies in the priority areas of Equitable Access to Care; Social Determinants of Health; Mental Health and Substance Use; and Chronic and Complex Conditions.
